Why are certain fields on my caregiver application Dynamic File not auto-populating with caregiver information?

Dynamic Files — such as caregiver applications (Initial Application, Complete Application, HHA Application, etc.) — are designed to auto-populate certain fields with data already stored in the caregiver's profile (e.g., name, address, phone number, SSN). If you notice that specific fields on one or more pages of an application Dynamic File are blank when they should be pre-filled, this typically means the auto-population mapping for those fields needs to be configured or corrected on the backend.

This is not something you can fix on your own as an end user. Here's what to do:

1. Identify the issue clearly. Note the exact Dynamic File name (e.g., "Complete Application"), the page number(s) where fields are not populating, and which fields are affected.

2. Take a screenshot showing the blank fields on the Dynamic File.

3. Submit a support request to Bolt HC Support with the Dynamic File name, page number(s), screenshot, and a link to the caregiver profile where you observed the issue.

4. Bolt Support will update the field mapping in the PDF Engine so that the correct caregiver data auto-populates into those fields.

5. Once the fix is applied, the fields will auto-populate going forward for all caregivers using that Dynamic File.

Note: This is a configuration adjustment handled by Bolt Support — it does not require any action on your part beyond reporting it. The fix applies globally to the Dynamic File template for your agency, so once resolved, it will work correctly for all caregiver records.
