How do I set specific fields on a Dynamic File as counter-sign fields?

Counter-sign fields on a Dynamic File are fields designated for an agency representative (such as an HR Admin or Manager) to complete and sign, rather than the caregiver. This is commonly needed on applications, orientation packets, tax forms, and other documents where certain fields (e.g., employer signature, supervisor name, date of review) must be filled out by agency staff after the caregiver has completed their portion.

Currently, designating fields as counter-sign on a Dynamic File is a configuration change that must be performed by the Bolt Healthcare support team. You cannot set counter-sign field assignments yourself.

To request this change:

1. Identify the specific Dynamic File (e.g., HHA/PCA Application, Orientation Packet, IT-2104, etc.) that needs counter-sign fields configured.

2. Identify the exact page(s) and field(s) that should be marked as counter-sign. Taking a screenshot and annotating the fields in question is very helpful.

3. Submit a support request to Bolt Healthcare, including:

- The name of the Dynamic File

- The page number(s) where the fields are located

- A screenshot highlighting which fields should be set as counter-sign

4. The Bolt support team will update the field designations on the Dynamic File so those fields are reserved for the counter-signing party.

Once the change is applied, caregivers filling out the document (e.g., via the Caregiver Portal) will not be able to edit the counter-sign fields. Instead, those fields will only be available for the designated agency staff member to complete during the counter-sign step.
