How do I get my agency's employer information (name, address, EIN) auto-populated on tax and employment forms like the W-4, I-9, or IT-2104?
Bolt can configure your Dynamic Files (such as the W-4, I-9, IT-2104, and other tax/employment forms) to automatically pre-fill your agency's employer information — including employer name, business address, and EIN — so that caregivers do not need to enter this information manually and it remains consistent across all submissions.
This is a one-time configuration per form, per agency. To request this setup:
1. Submit a support request to the Bolt HC support team.
2. In your request, include the following details:
- The specific form(s) you want updated (e.g., W-4 Standalone, IT-2104 Standalone, Employment Application W-4, etc.)
- Your agency's full legal employer name
- Your agency's full business address (street, city, state, ZIP)
- Your agency's EIN (Employer Identification Number)
3. The Bolt support team will configure the specified Dynamic File(s) so that the employer fields are automatically populated for all future submissions.
Once configured, any caregiver completing that form — whether through the Caregiver Portal or within the platform — will see the employer information pre-filled. If your agency's information changes (e.g., new address or updated legal name), submit a new support request to have the auto-populated values updated.
Note: If you use multiple versions of a form (e.g., a standalone W-4 and a W-4 embedded within an employment application packet), each version may need to be configured separately. Be sure to specify all applicable forms in your request.
